Completing the Contract and Itemization Sheet

Your Item Information

• Brand - Please enter the manufacturer of your item (example…Louis Vuitton, Chanel, Gucci, etc.).
• Color – Please enter the color (or colors) of the item.
• Item Description – Please enter the description of the item including (if available):
  • Item type (example…necklace, handbag, scarf, shoes, etc.).
  • Where/when item was purchased.
  • Condition.
• Size (if applicable) – Please enter the item size.
• Suggested Price - Couture reserves the right to price any item at its own discretion. You may enter a suggested price if there is a specific minimum amount at which you wish to sell your item(s).
• Final Price - Please leave this field blank. The final price of your items will be agreed upon by you and Couture upon receipt of your consignment items.

• Name - Please enter your first and last name.
• Street Address - Please enter your street address including apt #, suite #, etc.
• City/State/Postal Code - Please enter your city, state and postal code of your shipping address.
• Telephone - Please enter the telephone number(s) that we can use to contact you.
• Email - Please enter your primary email address.
• Consignment Account # - Please leave this field blank. Your account number will be provided by Couture upon receipt of your consignment items.
• Consigner Signature/Date – Please sign and date both the consignment contract and the itemization sheet.
• Couture Representative Signature/Date - Please leave this field blank. A Couture Boutique representative will sign and date the consignment contract when your account number is assigned.
